Responsibilities

Develop care plans for the patient Complete initial assessment for care of the patient Provide case management from admission to discharge Join the AgeRight Care at Home team as a Home Health RN and take your career to new heights! As an AgeRight Home Health RN, you will provide skilled nursing care in the patient’s place of residence. Care is provided under a physician’s order and coordinated through quality practice, case management, and through all aspects of the nursing process. The Registered Nurse is an integral member of the interdisciplinary team and provides supervision of the Home Health Aide & Licensed Practical Nurse.
