About the role

The nurse is responsible for traveling to pediatric patients' homes to provide healthcare services, including medication administration and wound care. They must coordinate with a multidisciplinary care team and provide essential education to patients and their families.

We are looking for a Pediatric Home Care Registered Nurse to work for one of our clients, a multi-service home care organization (CHHA), for their pediatric patients in the Bronx, NY.

This is a permanent, full-time job. Monday - Friday, 38.5 hrs/week.

Sign-on Bonus of $15,000 paid after 15 months of employment.

In this role you would be responsible for traveling to a patient’s home to administer healthcare services and helping patients maintain their independence.

Caseload - 20-25 patients per week.

An ideal candidate would be a graduate of accredited school of nursing, and currently registered in New York State as a Registered Professional Nurse and have experience with pediatric population.

Responsibilities

include, but not limited to:

Assess and chart observations of the patient’s condition at each visit.

Complete evaluation tasks, including reviewing medication and vital signs.

Administer Physician-prescribed medication.

Dress or redress wounds and assess the healing progress.

Provide education to patients and families on proper home health care procedures and strategies.

Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.

Keep the patient comfortable through palliative care.

Recommend tools or devices that may improve the quality of life for the patient.

Requirements

Candidates must be a graduate of an accredited nursing school and currently registered as a Professional Nurse in New York State. Experience working with the pediatric population is required.
