This role coordinates care plans, admits patients, and collaborates with interdisciplinary teams to promote optimal health outcomes.Responsibilities:Participate in on-call duties per policy.Support the Quality Management program.Arrange necessary equipment, supplies, and services for patients.Continuously review patient care, including service utilization, skilled need, homebound status, documentation, communication, and discharge planning per CMS and Colorado guidelines.Complete initial OASIS patient assessments.Use assessment data to establish nursing diagnoses and develop care plans with patient/family input.Initiate and update plans of care as patient needs change.Provide skilled nursing procedures, medications, and treatments as prescribed by physician.Supervise LPNs and home health aides per regulations.Reassess patients with change in condition or hospital discharge per regulatory standards, update care plans.Identify and implement discharge planning needs.Maintain knowledge of state, federal, and local home health regulations.Prepare clinical notes and update physicians regularly (at least every 60 days).Communicate patient needs and condition changes to physicians; obtain orders as required.Collaborate with team members and area team leader.Educate patients and caregivers on self-care, medications, diet, and preventive health measures.QualificationsPosition Qualifications:Current Colorado RN License required, or compact covering ColoradoValid Driver's License & clean Motor Vehicle Record (MVR)One year nursing experience in home health preferredAdditional InformationBenefits:PAID training!Work - Life balance, no overnight services401k available for ALL staffSalary = Medical, Dental, Vision & SupplementalSalary = Paid Vacation and HolidaysPay:Salaried ($75,000-$85,000/year)$45 - $120 per visitwww.argushomehealthcare.com
