The Part-Time Care Manager – Social Isolation Grant provides individualized care management services to older adults participating in a grant-funded initiative designed to reduce social isolation and improve overall well-being. This position conducts assessments, develops personalized care plans, connects participants to community resources, and provides ongoing support through home visits and telephone outreach. The Care Manager also ensures compliance with grant requirements through accurate documentation, data collection, and outcome reporting.
Older adults experiencing social isolation often face increased health risks and reduced quality of life. This role plays a critical part in helping participants remain connected, access essential resources, and maintain their independence while supporting Bergen Family Center's commitment to improving the health and well-being of the communities we serve.
Part-Time, up to 25 hours per week
Typically Monday through Friday during standard business hours, with flexibility based on participant needs and program requirements.
Occasional early morning, evening, or community event hours may be required for organizational events.
Compensation •
$25.00-$30.00 per hour (or as determined by Bergen Family Center)
Compensation is commensurate with experience and qualifications.

Requires a valid NJ driver's license, reliable transportation, and bilingual proficiency in English and Spanish. A degree in Social Work, Human Services, or a related field and experience working with older adults are preferred.
