About the role

Coordinates continuum of care arrangements and performs discharge planning assessments for patients in a hospital and long-term care setting. Serves as the alternate Social Service designee, managing admissions and care conferences in the absence of the Medical Social Worker.

Summary

Under the supervision of the Manager of Social Services, performs duties as required by government regulation and accreditation standards for discharge planning in collaboration with other disciplinary, hospital setting. Also serves as the alternate Social Service designee, in the absence of the Medical Social Worker, for the Long-Term Care facility.

Responsibilities

  • (50%)*Discharge Planning Assessments.
  • Upon referral, coordinates continuum of care arrangements for patients.
  • This includes working with patients, significant others, physicians, community resources, and other disciplines in the process.
  • Completes a Social Service/Discharge Planning assessment to ascertain patient needs, goals, and resources; includes patient/significant others in this process.
  • Reassess patient needs as appropriate.
  • Maintains legible patient records in a timely and confidential manner.
  • The patient files contain all pertinent forms and related information in an organized and easily accessible manner.
  • Maintains daily patient log. (50%)*Community Resources and LTC Social Worker Designee.
  • Demonstrates an awareness of available patient resources and utilized these resources in an appropriate manner.
  • Patients are allowed freedom of choice in all situations where possible.
  • Community Liaison.
  • Actively participates in interagency teams and maintains confidentiality.
  • Meets regularly for educational, networking, and patient advocacy purposes with Social Service organizations.
  • Provides essential Social Services to Long Term Care residents in the absence of Social Worker.
  • This includes admission protocol, development of discharge plans, participation in care conferences, advocacy, and related documentation.
  • OTHER DUTIES AND RESPONSIBILITIES: Builds professional relationships with patient, families and the treatment team to address conflict resolution with positive outcomes.
  • Communicates teamwork, caring and compassion.
  • Provides excellent customer service in accordance with the mission, vision and values of the health system.
  • Provides for the age specific needs of the population served according to evidence based standards and organizational policies/procedures for the adult and older adult populations.
  • Other duties as assigned.

Certifications and Licensures

CPI: Crisis Prevention Intervention

Required within 3 months of hire or transfer. TEAM ADVANCED: Techniques for Effective Aggression Mgt

Required within 6 months of hire or transfer.

Required Education: High school diploma or GED is required

One year of college preferred.

Other Information

Experience, training and skills

  • Minimum of one year of college, preferably in social science or related area. Registered with the Michigan Department of Licensing & Regulation at the minimum level of social work technician or meeting the criteria to become registered within 12 months under supervision. Minimum of two years' experience in providing direct delivery of service to client population. Medical knowledge, or ability to acquire such within one year. Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner. Must follow MyMichigan Health and The Michigan Mental Health Code policies and mandatory educational competency requirements. MyMichigan Health is a technology driven organization and employees need to demonstrate competency in Microsoft Windows.   • CONDITIONS: Exposure to stressful situations, including those involving public contact as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

Requirements

Requires a high school diploma or GED, with a preference for college coursework in social sciences and at least two years of direct service experience. Must be registered or eligible for registration as a social work technician with the Michigan Department of Licensing & Regulation.
