About the role

The CNA provides direct patient care, including monitoring vital signs, assisting with activities of daily living, and documenting patient status. They collaborate with the care team to implement patient plans and report condition changes to the charge nurse.

1. Utilizes the nursing process, for the performance of care, and completes documentation in accordance with patient type which may include Administers and performs nursing assessment in accordance with the applicable patient type, which may include: Acute, Observation, ER, SNF and Outpatient.

Contributes to, monitors and implements patient’s plan of care.

Responds to patient needs in a timely fashion.

Recognizes changes in patient's condition and notifies charge nurse in a timely manner.

Serves as patient advocate.

Performs tasks within the scope such as but not limited to vital signs, ADLs, Glucose POC

2. Participates as an effective member of the Care Team.

Responds in a timely and effective manner to patient concerns and provides immediate interventions when possible.

Provides written communication of concerns to necessary departments for appropriate follow up.

3.

Adapts to changes in workload and completes assignments while maintaining quality care.

Initiates and performs duties which need to be completed although not directly assigned.

5.

Promotes teamwork through effective interpersonal relationships with all staff.

6.

Demonstrates initiative and flexibility in provision of care.

7.

Offers assistance to other team members as needed.

3. Observes the patient at regular intervals, and documents on work sheet or verbally, information the Charge Nurse should know.

1. 1. Reports to the Charge Nurse any observations and/or symptoms indicative of patient’s changes and adverse reactions to treatments and/or response of complaints in a prompt manner. 2. Reports care for assigned patients.

4. Helps to set priorities when delivering nursing care.

1. 1. Maintains knowledge of location for needed equipment and supplies. 2. Re-stocks supplies as needed. 3. Reports equipment needing repairs. 4. Cleans and stores equipment when no longer needed.

5. Perform and maintain documentation.

1. 1. Documents accurate and ongoing assessments of patient status that reflect nursing interventions, patient responses, teaching and status of outcomes at discharge in care tracker. 2. Completes shift end check prior to leaving duty.

Follows approved shift report format when communicating patient information.

2.

Identifies and recognizes abnormal symptoms/changes in patient's condition, established priorities and takes appropriate action.

Appropriately reports condition changes to Charge Nurse.

Communicates and collaborates effectively with other health team members regarding patient condition, satisfaction needs and recommendations for meeting identified outcomes.

7. Promotes and ensures patient and staff safety in all aspects of responsibilities.

1. 1. Utilizes proper body mechanics, transfer/lifting techniques to minimize injury to patient and self. 2. Implements infectious disease controls as appropriate including isolation procedures, proper use of hand washing and gloves.

8. Performs other duties as assigned.

Requirements

Candidates must be able to perform nursing assessments and tasks within the CNA scope of practice. Proficiency in infection control, proper lifting techniques, and effective communication within a clinical team is required.
