Key Responsibilities
Conduct initial nursing assessments and periodic reassessments for home care clients.
Develop, review, and update individualized plans of care.
Perform routine supervisory visits to monitor caregiver performance and ensure quality, compliant care.
Assess clients' health status, identify changes in condition, and recommend appropriate interventions.
Collaborate with physicians, caregivers, clients, families, and agency staff to coordinate care.
Complete accurate, timely electronic documentation and required reports.
Ensure compliance with Maryland home care regulations, agency policies, and professional nursing standards.
