Conduct comprehensive patient assessments and develop individualized care plans for home health clients Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting Monitor and evaluate patient health status, documenting changes and communicating with physicians as needed Educate patients and family members on disease management, medications, and home safety Coordinate care with interdisciplinary team members including therapists, aides, and case managers Ensure accurate and timely clinical documentation in compliance with home health regulations Provide compassionate end-of-life and chronic disease management support as needed
