About the Role

Community Support Team is a direct and indirect periodic service in which the team provides direct intervention and also arranges, coordinates, and monitors services on the behalf of the recipient. This service is provided in any location. Community Support services also include telephone time with the individual recipient and collateral contact with the person who assists the recipient in meeting his/her rehabilitation goals. Key Responsibilities: Assists with crisis interventions Assists the Team Lead with behavioral and substance use disorder treatment interventions Assists with the development of relapse prevention and disease management strategies Participates in the initial development, implementation, and ongoing revision of the PCP Communicates the individual’s progress and the effectiveness of the strategies and interventions to the Team Lead as outlined in the PCP Provides intensive case management Linkage and referral to formal and informal supports Monitoring and follow up Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services This description outlines the general scope and level of work for this role.   • $22.91 - $29.42 Placement within the range is based on licensure level, education, and experience.   • Professionals must meet one of the following: Bachelor's degree from an accredited university in a Human Services field with 2 (two) years experience working with the population to be served. Bachelor’s Degree in a non-Human Service field with 4 years full-time, post-bachelor’s degree experience with the population served. Master’s degree in human services with 1 year experience working with population served. Valid driver’s license, auto insurance, and reliable transportation Must complete the state mandated Community Support Team training within specified timeframes. Requirements
