The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

We are seeking a part-time or full-time Nurse Practitioner or Physician Associate/Assistant to work with MNHC adult populations. This position reports to the Chief Medical Officer and is exempt role with a salary range of $151,376 to $166,015 (1 FTE) Based on years of experience.

New grads are welcome to apply and upon hire, are welcome into our *Puente Program, which offers mentorship, preceptorship, and full support during the onboarding and acclamation period.

Primary objective

The Nurse Practitioner or Physician Associate will work under the general direction of the Lead Physician and the Chief Medical Officer. The provider will perform comprehensive physical examinations and document findings using the problem-oriented record system. Based on examination results and in accordance with standardized procedures and evidence-based guidelines, the provider will plan and implement appropriate treatment for each patient. This role supports standardized clinical processes related to patient-centered medical home, population-based patient management, patient and family self-care, and other quality measures.

Responsibilities

  • Provide comprehensive primary care, including preventive care, chronic disease management, and acute care.
  • Conduct medical assessments, develop treatment plans, and coordinate referrals.
  • Collaborate with a multidisciplinary team, including nurses, behavioral health providers, and care managers.
  • Maintain accurate documentation in the Electronic Health Record (Epic preferred) to ensure continuity of care.
  • Participate in quality improvement initiatives to enhance patient outcomes.
  • Advocate for health equity and culturally responsive care.

Minimum Qualifications

  • Nurse Practitioner or Physician Assistant certified by a Family, Adult/Geriatric Program.
  • Valid California and DEA License
  • Fluent in Spanish.
  • Passion for community health and serving diverse populations.
  • *ABOUT PUENTE The Puente Program is a one-year transition for new-grad NPs and PAs or those with less than one year of experience. You get a dedicated preceptor, weekly consultation time, online education, a slower ramp-up, coaching, and provider wellness, all designed to help you build confidence and thrive in community health. As a condition of employment, all candidates are required to meet specific immunization standards. Documentation of current immunizations, including Varicella, TDAP, COVID-19, MMR, and Hepatitis B, as well as TB test results valid within the last two (2) years must be submitted before the start date. Requirements

Must be a certified Nurse Practitioner or Physician Assistant in Family or Adult/Geriatric programs and be fluent in Spanish. While three years of experience is preferred, new graduates are welcome through the Puente Program.
