  • Location: Whitesburg, GA Description Caring Hearts of Georgia Health Services is seeking a dependable, compassionate, and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support our clients and caregivers in the home care setting. The LPN will help ensure clients receive safe, high-quality care by completing supervisory visits, assessing client needs, documenting changes in condition, communicating with leadership, and providing guidance to Personal Care Aides and Certified Nursing Assistants. This position also include direct client care and skilled nursing support for clients with higher-level needs, including personal care, tracheostomy care, catheter care, and ventilator care. Applicants must have experience and confidence providing care in the home or community setting while maintaining client safety, dignity, and comfort. The ideal candidate is professional, dependable, responsive, and comfortable working independently in the community. This individual must demonstrate sound clinical judgment, timely documentation, respectful communication, and a genuine commitment to protecting the health, safety, and dignity of every client.

Responsibilities

Assess client needs and report changes in condition promptly Assist with care planning and client service updates as needed Complete accurate and timely documentation Provide personal care support when assigned Provide tracheostomy care as ordered and within scope of practice Provide catheter care and monitor for concerns Provide ventilator-related care and observation for clients requiring respiratory support Communicate professionally with clients, families, caregivers, and office staff Follow company policies, client care plans, and applicable state regulations Promote client safety, dignity, infection prevention, and quality care

Current and unrestricted Georgia RN or LPN license Must provide Georgia license/certification number during the application process Current CPR/BLS certification Valid driver’s license Reliable transportation Current auto insurance Experience with personal care required Experience with tracheostomy care required Experience with catheter care required Ventilator experience required Strong communication and documentation skills Ability to work independently and travel to client homes Home care, long-term care, geriatric, private-duty, or facility experience preferred
