Physical Therapist

Responsibilities

Provide physical therapy evaluations and develop individualized treatment plans for adult and geriatric clients. Assess mobility, strength, balance, endurance, gait, transfers, range of motion, and overall functional status. Implement evidence-based interventions including therapeutic exercise, gait training, balance training, fall prevention, and functional mobility programs. Utilize integumentary protective interventions to prevent wound formation and promote healing when appropriate. Collaborate with nurses, therapists, physicians, caregivers, and other healthcare professionals to coordinate patient-centered care. Continually assess patient progress and modify treatment plans to maximize functional outcomes. Educate patients, family members, and caregivers on exercises, safety strategies, adaptive techniques, and disease management. Complete timely and accurate documentation utilizing electronic medical records and point-of-care technology.

Qualifications

Current New Jersey Physical Therapist (PT) license. Minimum of one year of recent experience as a Physical Therapist. Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA). Home health experience preferred but not required. Strong assessment, clinical reasoning, and communication skills. Ability to work independently and effectively manage time and priorities. Proficiency with electronic medical records and technology.
