What You'll Do As a Home Care Coordinator, you'll serve as a key link between physicians, hospitals, patients, caregivers, and our clinical team to ensure a seamless care experience.
• In this role, you will:
• Coordinate communication between providers, patients, and clinicians.
• Support medication verification and plan of care coordination.
• Assist with scheduling, care coordination, and other administrative support that helps our clinicians deliver exceptional care.
• Collaborate with your team to provide the highest quality experience for every patient.
• We’re looking for someone who:
• Has a clinical background (Licensed COTA or PTA).
• Enjoys organization, communication, and problem-solving.
• Is detail-oriented and compassionate.

• QualificationsExperience: 2 years of medical office experience and/or direct patient care. Ability to multi-task in a fast-paced environment. Ability to work and communicate professionally with patients, medical partners, employees, and supervisors.

Required

Current Minnesota PTA/LPN/COTA license in good standing. Knowledge of medical terminology Good judgment and decision-making skills. Strong attention to detail. Technologically proficient and driven by accuracy. Regular attendance and flexibility with scheduling necessary.
