Responsibilities

Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults Help patients restore functional mobility and safely perform daily activities with greater confidence and independence Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications
