About the role

The Hospice RN Case Manager coordinates and provides compassionate end-of-life care for terminally ill patients and their families in home and community settings. Hospice RN Case Manager Small Caseloads, Meaningful Relationships, Extraordinary Lifestyle Balance At Grand County Hospice, a program of Moab Regional Hospital, we provide what our team calls boutique hospice care—deeply personal, relationship-centered care that allows nurses the time and flexibility to truly support patients and families during one of life's most important transitions.
