The UT Health ET Home Health Physical Therapist plays a critical role in delivering high-quality, patient-centered physical therapy services within the home health care setting.

This position focuses on assessing, planning, and implementing individualized treatment plans to improve patients' mobility, function, and overall quality of life.

The therapist will work closely with patients recovering from illness, injury, or surgery, providing therapeutic interventions that promote independence and prevent further disability.

Collaboration with interdisciplinary teams, including physicians, nurses, and social workers, is essential to ensure comprehensive care coordination.

Ultimately, this role aims to support patients in achieving optimal health outcomes while maintaining safety and comfort in their home environments.

Position Summary

Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

As a Physical Therapist, you will

Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.

Requirements

  • Successful completion of accredited physical therapy program and state licensure exam.
  • Valid Physical Therapy license
  • 1 year of home health experience preferred
  • CPR certification
  • Valid Driver’s license
  • Valid Auto Insurance
  • HomeCare HomeBase experience preferred
  • Physical Demands
  • Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
  • Must be able to lift, carry, pull, or push up to 100 lbs..
