About the role

Provide high-quality, patient-centered care to residents in skilled nursing facilities through daily rounding and treatment planning. Coordinate with facility nursing staff and ensure compliance with clinical and regulatory standards.

We are currently seeking a full-time Advanced Practice Provider in DeKalb, IL to serve local skilled nursing facilities within a close-knit, mission-driven care community.

Exceptional compensation, with the opportunity to exceed industry benchmarks The estimated compensation range for this position is $104,000 to $169,000 per year, based on expected earnings under our uncapped productivity-based compensation model.

This role is eligible for a longevity bonus of up to $50,000 and additional quality bonus programs.

Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.

Communicate with facility nursing staff via phone or secure messaging as needed.

Maintain reliable on-site attendance for scheduled shifts.

Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your Qualifications Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).

Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Requirements
