About the role

Manage a caseload of up to 40 recipients by providing monthly visits, crisis intervention, and support for financial resource management. Coordinate with other providers to ensure consistency of services and maintain detailed case records in compliance with regulations.

Catholic Charities of Broome County Community Health Services Protective Services for Adults Care Manager Level 2

Grade 8 $19.95-$22.45/hr. FLSA Non-Exempt I.

Qualifications

A. Education

1. Minimum associates degree in human services or related field. 2. Bachelor’s degree in a human services field is preferred.

B. Experience

1. ​​​​Minimum of one year experience for bachelor’s degree and two years for associates degree working in providing direct social work service to special needs individuals including those living with a mental illness, developmental disability, substance use or elderly.

2. Knowledge of community mental health services and services available to developmentally disabled and geriatric.

  • Excellent communication skills, both oral and written.
  • 2.
  • High degree of organizational skills to meet assigned work demands.
  • 3.
  • Demonstrated ability to work with a team as well as function independently.
  • 4.
  • Good computer skills with working knowledge of Microsoft Office necessary.
  • 6.
  • Knowledge of special needs individuals including, mental illness/psychiatric disorders/developmentally disabled and geriatrics.
  • 7.
  • The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. or less.
  • 8.
  • A valid driver’s license and the ability to legally operate a vehicle in NYS.
  • 9.
  • Regular and substantial contact with those individuals receiving services.

  • Assist recipients in learning to use financial resources responsibly, including assistance with budgets.
  • 11.
  • Provide crisis intervention services as necessary.
  • 12.
  • Promote self-advocacy and personal empowerment.
  • 13.
  • Maintain case records, progress notes and assessments, as required in compliance with Agency, County and Office of Children and Family Services regulations.
  • 14.
  • May carry caseload of up to 40 recipients and provide a minimum of one visit per month.
  • 15.
  • Complete and submit monthly reports related to services provided and individuals served.
  • 16.
  • Encourage medication, education and compliance.
  • 17.
  • Communicate regularly with other providers and individuals to develop or enhance consistency of services.
  • 18.
  • Coordinates Representative Payee services as needed.
  • 19.
  • Participate in ongoing training, attend meetings as required.
  • 20.
  • Physical condition commensurate with the demands of the position.
  • 21.
  • Perform other related duties as required.
