About the role

The SLP provides skilled speech therapy services to patients in senior living communities based on physician orders. Responsibilities include conducting evaluations, establishing plans of care, and coordinating with healthcare teams to enhance patient independence.

Overview We are seeking a Speech Language Pathologist (SLP) to provide skilled physical therapy services to patients across Senior Living Communities.

As a Speech Language Pathologist (SLP), you will administer speech therapy to patients on an intermittent basis in their place of residence.

This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist.

Responsibilities

  • Provides full range Speech Language Pathology Services as ordered by the physician.
  • Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.
  • Identifies functional speech defects and establishes a plan of care to improve the patient’s function.
  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommends communication devices/aids as indicated.
  • An initial evaluation, including a plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by a physician every 60 days is necessary if further treatment is to be continued. A progress note is written at each visit. A reevaluation is written when the expected duration of treatment is reached. A summary is written upon the patient’s discharge.
  • Helps develop the plan of care and revises as necessary.
  • Consults with physicians regarding change of treatment.
  • Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participates in case conferences, in-services, and other meetings required to ensure coordinated and comprehensive care.
  • Prepares and submits clinical and progress notes per organization policy.
  • Maintains clinical competency in speech-language pathology practice and theory.
  • Identifies patient and family/caregiver needs for other services and refers as appropriate.
  • Instructs patient, family/caregiver, and other organization health care personnel in the patient’s treatment regimen as indicated.
  • A supervisory visit is made every fourteen days on each patient seen by a home health aide as assigned.
  • Therapist will complete mandatory CEU’s to maintain professional licensure.
  • Understands and adheres to established policies and procedures.
  • Therapist will complete Christian Care onboarding as well as Covenant Home Health orientation.

Qualifications

Possesses a master’s or doctoral degree in speech-language pathology (SLP) Is licensed as a speech-language pathologist by the State in which the individual furnishes such service, or A person who has completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience, and has successfully completed a national examination approved by the Secretary. A minimum of one (1) year of clinical experience in Speech-Language Pathology. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements. Schedule and service areas: Full-time & PRN to cover the West Valley Part-time PRN to cover the East Valley

Requirements
