Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
Conduct
Comprehensive
Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.
Coordinate
Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.
Monitor and
Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.
Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.
Advocate for the
Patient: Provide education, resources, and emotional support to patients and their families to promote independence and wellness.
RN (Registered Nurse) with active (state) license.
At least 1 year of experience in home health or case management preferred.
