We’re looking for a Home Health Case Manager who’s organized, clinically skilled, and ready to join a supportive team that values autonomy, teamwork, and work-life balance.

What You'll Be Doing Manage the Care Journey

Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.

Conduct

Comprehensive

Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.

Coordinate

Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.

Monitor and

Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.

Documentation

Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.

Advocate for the

Patient: Provide education, resources, and emotional support to patients and their families to promote independence and wellness.

What We’re Looking For License

RN (Registered Nurse) with active (state) license.

Experience

At least 1 year of experience in home health or case management preferred.

Skills

  • Strong critical thinking, communication, time management, and the ability to work independently.
  • Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
  • Reliable
  • Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle. Pay per visit rates starting at $65
