Provides general observational support and companionship for patients across various age groups to ensure safety. Responsibilities include maintaining constant one-to-one observation and alerting nursing staff to behaviors that could lead to injury.
Provides general observational support for assigned patient care area. Works with other members of the patient care team to deliver care to patients in age groups including children, adolescents, adults, and geriatrics based on unit population.
Advanced Certification or training (i.e. LPN, Crisis Training, Tech) Constant Observation or One to One observation experience preferred, but not necessary.
HS Diploma or Equivalent preferred.

Functions:
Provide companionship to the patient.
Alert the RN of patient behavior which could lead to injury.
Maintain One to One observation status (constant eye sight and within arm's reach at all times) of the patient.
Maintain Special Observation documentation form (patient's location every 15 minutes) if determined suicide precautions/one to one observation.
Provide de‐escalation tactics to help control potential stressful situations.
Communicate patient's needs to patient care team.
Maintain and respect the patient's responsibility, privacy, and dignity.
Demonstrates behavior appropriate to keeping the patient relaxed.
Participates in performance improvement activities to improve service and care. Pay Range $17.47 - $24.26/hour, DOE #LI-MC1
