Greater Seacoast Health is seeking a compassionate and dedicated Social Worker to become part of the founding care team at our brand-new Wakefield Primary Care practice.

As an integrated member of our healthcare team, you'll work alongside providers, nurses, behavioral health clinicians, and care coordinators to help patients overcome barriers to care, improve their health, and connect with the resources they need to thrive.

As a Social Worker, you will

Conduct psychosocial assessments and identify barriers to care.

Connect patients with housing, transportation, financial assistance, food resources, and community services.

Provide advocacy and care coordination for patients with complex social and behavioral needs.

Support patients experiencing crisis or significant life challenges.

Collaborate with providers and interdisciplinary care teams to develop patient-centered care plans.

Maintain timely and accurate documentation within the electronic health record.

Coordinate referrals and promote continuity of care.

Participate in quality improvement initiatives and integrated care team meetings.

✔ Bachelor's degree in Social Work (BSW) or a related human services field.

✔ Strong communication, organizational, and interpersonal skills.

✔ Ability to work effectively within a multidisciplinary healthcare team.

✔ Commitment to providing compassionate, patient-centered care.
