Description TEAM is looking for a Care Coordinator to provide outreach, screening, assessment, and advocacy to enhance the quality of life for the clients we serve.

Our multidisciplinary team provides personalized, whole-person care through behavioral health, nursing, nutrition services, health coaching, and care coordination.
We are committed to helping members improve their health through education, prevention, and meaningful lifestyle change.
The position is Monday-Friday, Full Time/In-Office.
There may be some occasional travel for client trainings and presentations.
The Care Coordinator’s salary is $50,000 -$55,000/year, commensurate with education and experience.

Bachelor’s degree from accredited four-year college or university with a major in social work, psychology, sociology, or Licensed Practical Nurse, or closely related field. 1 year experience in social services, hospital work, clinic work, utilization review or care coordination in a public or private sector.

Requires a Bachelor's degree in social work, psychology, sociology, or a nursing license, with at least 2 years of patient care experience. Proficiency in Microsoft Suite and EMR software is required, and bilingual skills in Spanish are preferred.
