Job Summary and Responsibilities As our Social Worker, you'll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise.

By addressing social, emotional, financial, and discharge planning needs, you'll help remove barriers to care and support positive patient outcomes throughout the care journey.

Every day you will collaborate with patients, families, physicians, nurses, and the interdisciplinary care team to develop individualized plans of care and coordinate safe, timely transitions.

You'll serve as a trusted advocate, connecting patients with community resources and support services that promote continued health and well-being beyond the hospital.

To be successful in this role, you will bring strong clinical assessment, communication, and problem-solving skills along with a passion for patient advocacy.

You'll thrive in a collaborative environment where your ability to build relationships, navigate community resources, and support patients through challenging situations makes a meaningful difference every day.

Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.

Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and facilitate timely referrals to appropriate post-acute and community resources.

Advocate for patients and families by connecting them with support services and assisting with applications for available community and financial resources.

Educate patients, families, and healthcare team members on available services, care options, and community resources to support successful transitions of care.

Document assessments, interventions, and care coordination activities while maintaining compliance with organizational, state, and federal standards.

Job Requirements Certified Social Worker (CSW) in Nebraska upon hire Minimum of a bachelor's degree in Social Work, prefer master's degree in Social Work Basic Life Support (BLS) from the American Heart Association upon hire

Requirements

Requires a bachelor's degree in Social Work and CSW certification in Nebraska upon hire. Basic Life Support (BLS) certification from the American Heart Association is also required.
