About the role

Provides clinical mental health assessments and manages a caseload of high-risk members with complex medical and behavioral needs. Conducts psychosocial interventions and transitions of care to support a holistic population health treatment plan.

Schedule Full Time, Day Shift (80 hours biweekly) Hours

General Summary

Provides quality clinical mental health assessments and consultation to providers and supporting clinical care team staff as necessary. They are responsible for triage and management of patients with clinical issues related to their behavioral or psychosocial wellbeing. Additionally, the behaviorist conducts psychosocial assessments, interventions, Transitions of Care (TOC), education, and referrals to inform a holistic treatment plan for patients, in support of Population Health. They are responsible for the management and oversight of a caseload of moderate - high risk Members with complex medical/behavioral /psychosocial needs.

Requirements

Requires a Master's Degree in Social Work and a Licensed Clinical Social Worker (LCSW) certification upon hire. Prefers at least 2 years of clinical experience in care management or behavioral health and experience with substance abuse treatment.
