Home Health Physical Therapist (PT): $80,000–$120,000 plus weekly production bonuses

This is a home health Physical Therapist role, not a clinic slot. You treat one patient at a time in their home, control your day, and get paid for production. Interim HealthCare of West Michigan is locally led and employee-owned.

- Salary range $80,000–$120,000, plus weekly bonuses you control with your caseload

- Plan (ESOP): the company contributes shares for you at no cost. Eligible after 1 year and 1,000 hours.
The work you'll do as a Physical Therapist
- Typical caseload is one-to-one home visits across Kalamazoo and nearby communities (Portage, Oshtemo, Comstock, and surrounding towns)
- Evaluate, treat, and progress patients after joint replacement, illness, fall, stroke, or deconditioning
- Build the plan of care with nursing, OT, and SLP
- Educate the patient and family so the plan actually gets done between visits
- Document in the EMR the same day
You are not double-booked. You are not stuck in a gym bay. You do need a reliable vehicle and a clean driving record.
Who this is Physical Therapy opening for
- Active Michigan PT license (or board-eligible and already scheduled to test)
- Graduate of an accredited PT program
- CPR
- Valid driver’s license, auto insurance, and a reliable vehicle
- Home health experience is a plus, not a requirement
- New graduates are considered if you want home health and will complete our field orientation with a preceptor
