About the role

Organizes and conducts occupational therapy programs to facilitate the rehabilitation of patients with physical or psychosocial disabilities. Responsibilities include performing evaluations, developing treatment plans, and educating patients and caregivers on health and prevention.

What You Will Do: Organizes and conducts Occupational Therapy program to facilitate the rehabilitation of people with physical and/or psychosocial disabilities.

The Physical Therapist directs the patient's participation in selected activities/treatments to restore, re-enforce and correct pathology to promote and/or maintain good health.

Performs evaluation(s) and develops an effective treatment plan appropriate to a patient’s rehabilitation needs within the care continuum.

Performs effective treatment techniques in a competent manner.

Educates patient/family/caregiver in applicable health information, treatment and prevention.

Initiates and participates in the discharge planning process.

Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.

Performs other duties as assigned.

Complies with all policies and standards.

For specific duties and responsibilities, refer to documentation provided by the department during orientation.

Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.

Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Schedule and Territory: Full time, 40 hours per week.

Monday-Friday with rotating weekends and holidays 100%25 travel to the patient's home for care Eastern Cuyahoga County.

The territory for this position will include: Cleveland, Garfield, Shaker Heights, Euclid, Richmond Heights, Cleveland Heights, Mayfield Heights, Beachwood, Pepper Pike, Lyndhurst, University Heights, South Euclid, Bedford, Solon, Maple Heights, Twinsburg, Hudson, Chagrin Falls, Macedonia, Northfield and Pennisula.

Education and Certifications

Graduate of an accredited Occupational Therapy program required. Evaluation and treatment skills necessary to provide Occupational Therapy appropriate to the developmental and chronological age of patients on caseload. Professional initiative, objectivity, good time management and interpersonal skills. Physically, cognitively and perceptually able to provide patient treatment and able to assist in physical mobility of patients and equipment. Basic Life Support (BLS) Required within 90 Days. Is awaiting Ohio licensure status of Limited Permit required. Licensed by the State of Ohio to practice Occupational Therapy required.

Requirements

Requires graduation from an accredited Occupational Therapy program and licensure by the State of Ohio. Basic Life Support (BLS) certification is required within 90 days of hire.
