Garden City Health Care Center is seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to join our dedicated rehabilitation team. Whether you are an experienced clinician or a newer graduate, we offer a supportive, collaborative environment where your work truly makes a difference in residents’ independence and quality of life.

General purpose

The Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist with patient-related activities and provides direct patient care in accordance with physician orders, treatment plans, and applicable regulations.

  • Essential duties and responsibilities
  • Treat residents as directed by the supervising Occupational Therapist
  • Implement treatment plans according to physician orders
  • Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per OT Board, facility policy, and payer requirements
  • Communicate resident progress, problems, and plans with the supervising OT and interdisciplinary team
  • Assist nursing staff with training of Restorative Aides as appropriate
  • Participate in in-service training programs and care conferences
  • Educate residents, families, and caregivers on maintenance programs and discharge planning
  • Participate in discharge planning and transition of care
  • Maintain a clean, safe, and organized treatment area
  • Report equipment issues to ensure safe and effective operation
  • Maintain positive, professional relationships with residents, families, and staff
  • Supervisory responsibilities

Qualifications

  • Education & Experience
  • Graduate of an accredited Occupational Therapy Assistant program
  • Skilled nursing or geriatric experience preferred
  • Licensure & Certifications
  • Current, active California Occupational Therapy Assistant (COTA) license
  • Must meet all continuing education requirements
  • Must maintain license in good standing with the State Board
  • Skills
  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
  • Strong communication, organization, and problem-solving skills
  • Ability to work independently and as part of a collaborative rehab team
  • Physical demands
  • Frequent standing, walking, reaching, pushing, and pulling
  • Ability to exert up to 50 pounds frequently and 100 pounds occasionally
  • Occasional climbing, balancing, stooping, kneeling, crouching, or crawling
  • Visual abilities include close, distance, color, peripheral, and depth perception
  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions

Requirements

Candidates must be a graduate of an accredited Occupational Therapy Assistant program and hold a current California COTA license. Experience in skilled nursing or geriatric care is preferred.
