  • You’ll Do: As a Nurse Practitioner at CleanSlate, you’ll play a vital clinical role in treating patients with opioid and alcohol use disorders through medication-assisted treatment (MAT). You’ll work alongside a team of dedicated medical and behavioral health professionals to ensure high-quality, patient-centered care.

  • Responsibilities: Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi Evaluate patients and determine individualized treatment plans Monitor patient progress and adjust medications as needed Collaborate with physicians and care teams on complex cases Maintain accurate documentation in the EMR within 48 hours Participate in clinical meetings, training, and quality improvement efforts Support telemedicine and occasional regional coverage as needed

Requirements

Requires an active Nurse Practitioner license, DEA registration, and a graduate degree from an accredited NP program. Experience in addiction medicine or behavioral health is preferred.
