Monday-Friday 8-1630, weekends based on department needs Conducts new patient interviews and assesses patient's function health and educational needs. Monitors patient's lab values, test results and progress. Provides patient/family education. Coordinates patient discharge needs to ensure smooth transition from hospital to outside agencies Additional Preferred Requirements Masters Degree Specialty certification, Certified Heart Failure Nurse or Cardiac Nurse Specialist Strong cardiology or ICU background

Northwest HealthCare, six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services.

The Nursing Administration oversees the various Nursing departments and provides clerical support.

Preferred Qualifications

Role Purpose Acts as a patient advocate by providing education, support and interventions throughout the continuum of patient care including pre & post hospitalization. Performs duties by working with physicians to identify and carry out treatment plans.

Responsibilities

  • Works with the Medical Director and other physicians to identify and carry out the treatment plan. Acts as a Patient Advocate helping to navigate patients through the diagnosis, treatment and rehabilitation process. Serves as a resource to assist patients and families, other professionals and the community. Provides education for the community and referring physicians. Facilitate clinical protocols established by Medical Director(s). BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job. Minimum Requirements Education Bachelor's Degree

Requirements

Requires a Bachelor's Degree in Nursing and a valid RN license with less than 2 years of experience. A Master's degree and specialty certifications in Heart Failure or Cardiac Nursing are preferred.
