North Valley Specialty Group (NVSG) is a growing mobile wound care practice serving skilled nursing facilities and homebound patients across Northern California. We are seeking a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to deliver advanced wound care in the field, supported by a dedicated wound care technician and a full administrative team handling scheduling, billing, and documentation support.
Perform advanced wound care treatments, including debridement and application of skin substitute grafts, for patients in skilled nursing facilities and home settings Evaluate wounds, develop treatment plans, and monitor healing progress across your patient panel Document visits accurately and timely in the EMR Work alongside a dedicated wound care technician who assists with supplies, setup, measurements, and charting support Coordinate with facility nursing staff, primary care providers, and families Travel to patient locations throughout the Sacramento region on a structured daily route What We're Looking For
Active, unrestricted California NP or PA license (required) Current DEA registration (or eligibility) Minimum 1 year of clinical experience; wound care experience strongly preferred Debridement and skin substitute graft experience a plus Experience with SNF or home-based care a plus Comfortable working independently in a mobile, field-based environment Professional, reliable, and an excellent communicator with facility staff, patients, and families Valid California driver's license and reliable transportation Compensation
$120 per completed patient treatment visit Typical providers see 10 to 14 patients per day (estimated $240,000 to $340,000 annually at full schedule) Uncapped earning potential; your income grows with your route Technician support on every visit so you can focus on clinical care Schedule
