The nurse plans organized and directed services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individuals and families within their homes and communities.
1. Prepare clinical notes and updates the primary physician when necessary 2. Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains physician’s orders as required 3. Communicate with interdisciplinary team
Candidates must be a graduate of an accredited nursing school with a current state LPN license and CPR certification. At least one year of clinical nursing experience is required, along with a valid driver's license and insured vehicle.
