Schedule: Monday- Friday with occasional on-call
The Home-Based Primary Care Physician provides direct patient care in patients’ homes, assisted living facilities, and personal care facilities. This physician also serves as a clinical supervisor and subject matter expert for home-based primary care nurse practitioners practicing in the same settings. Responsibilities include evaluating and treating chronic conditions and urgent episodic illnesses and injuries; ordering and interpreting selected laboratory and diagnostic tests; making referrals to specialists as needed; prescribing medications; and performing procedures within the physician’s scope of practice, established protocols, and available equipment. Position Qualifications: M.D. or D.O. required. Current licensure, or eligibility for licensure, to practice medicine in Kentucky, including DEA registration and controlled substance authority, required. Fellowship training in Geriatrics is preferred but not required. Board certified or board eligible, with board certification expected within five years of employment, required. Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
