The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
30am - 5:00pm
Location: Marin County with additional coverage to Sonoma County as needed
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Current license as a registered professional nurse in the State of California.
Public Health Nursing certification highly desirable.
HPNA certification preferred.
$58.34 - $72.95 hourly

Requires a current California Registered Professional Nurse license and either one year of professional nursing experience or a BSN. Experience in acute care, home health, or hospice settings is preferred.
