About the role

Provide direct nursing care and medication management for mentally ill patients in an outpatient behavioral health clinic. Coordinate with pharmacies, prescribers, and interdisciplinary teams to ensure comprehensive patient treatment and rehabilitation.

Description Summary/Objective

Work in an outpatient behavioral health clinic which provides care and treatment to patients who are mentally ill. Work as a member of an interdisciplinary treatment team that provides well-rounded medical care for the whole person. Perform practical nursing care and related technical work in the care, treatment and rehabilitation of mentally ill individuals, coordination with pharmacies, internal and external medication prescribers, and other clinical staff. The LPN provides general instructions; performs work independently, receiving oversight by the Registered Nurse supervisor and/or the Psychiatric Nurse Practitioner. The LPN will receive periodic review of work to ensure that acceptable practices are followed; and will perform other related duties as assigned by Clinic management.

Essential Functions

  • · Provide a variety of direct nursing care to clients. · Provide patient education about medications and side effects. · Administer injections as directed by prescribers. · Provide nursing care as needed, such as vital signs (weight, blood pressure, blood sugar, etc.). · Coordinate the filling and refilling of prescriptions for clients. · Organizing and administering medications to clients in respite programs. · Assemble, label, and log medication samples to provide to clients. · Cross-check charts for refills. · Request refills from prescribers if needed. · Contact pharmacies to arrange for refills. · Coordinate insurance company requests for pre-approval. · Respond to prescriber requests for client information. · Liaison with various treatment providers – pharmacies, hospitals, case managers, prescribers, primary medical providers, and families of consumers. · Create effective rapport with all contacts, internally and externally · Advocate for client appointments when needed · Research and confirm insurance status. · Complete all clinical and administrative documentation work in a timely and clinically-appropriate manner. · Assure confidentiality is always maintained. · Required to have a valid NYS driver’s license and ability to drive legally.

  • Environment: Office environment will require occasional travel to recruitment events, conferences and/or meetings and moderate travel within the community to include off-site/in-home visits. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments.

Equipment

This position is required to maintain a working knowledge of related office equipment including personal computers and printers, audio-visual equipment, telephone systems, copiers, fax machines, etc.

Physical Demands

Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Requirements

Position Type/Expected Hours of Work: Full-time, traditional and non-traditional hours required. This position is required to work on-site.

Travel

Travel is required throughout service area (Jefferson, Lewis, Oswego, and St. Lawerence County).

Required Education and Experience

Licensed practical nurse (LPN) current NYS License. Prior nursing experience with children/adolescents preferred. CPR and First Aid certified preferred.

Skills/Abilities/Knowledge

Strong organizational abilities and office skills are essential for this position. This person must also have excellent people skills and be a team player. Ability to work with diverse populations. Must demonstrate effective and excellent oral and written communication skills and the ability to work cooperatively with others. Must be detail-oriented and possess excellent time-management skills. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Able to react to change productively and handle other duties as assigned.

Additional Eligibility Qualifications

Must maintain privacy of confidential information. Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks. These requirements are to include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and your eligibility to work in the United States. A valid NYS driver’s license required. Corporate Compliance, HIPAA Privacy Security, and Workplace Harassment and Discrimination Training will be required following date of hire.
