The Grant-Funded Healthy Start Case Manager is responsible for delivering comprehensive case management services to eligible prenatal, postpartum, and parenting families enrolled in Hamilton Health Center’s Healthy Start program. Healthy Start serves pregnant women and families with children up to 18 months of age who reside in Dauphin County. The Case Manager works to improve access to medical care and community resources, promote healthy behaviors before, during, and after pregnancy, and support safe and healthy infant and toddler care practices through a patient-centered, community-based approach.

Bachelor’s degree in Social Work, Psychology, Sociology, or a related field, preferred.
Candidates with a bachelor’s degree in a non-related field, or a combination of relevant education and experience related to the functions of the position, will be considered.
Prior CPR and/or First Aid certification required or ability to obtain upon hire.
Valid driver’s license with the ability to travel throughout the service area.
• Experience performing case management or comparable social service functions, preferred.
