Mt Lebanon Blvd - Pittsburgh, PA 15234About Chartiers Center Chartiers Center is a private, non-profit behavioral health care organization serving Southwestern Pennsylvania. Our mission is to provide compassionate support to those in need who face behavioral health, co-occurring disorders or intellectual/development disability (IDD) challenges as they transform our lives. As a Blended Service Coordinator with Chartiers Center, you would provide support and service coordination to adults living with serious mental illness and co-occurring conditions, including substance use disorders, physical health concerns and intellectual disabilities. This is a highly rewarding community-based role where you build meaningful relationships, advocate for individuals and help them achieve greater stability, independence, and recovery.

Job duties include: Meeting with individuals in homes and community settings. Conduct assessments and identify strengths and needs and goals. Develop, implement and monitor individualized service plans. Connect individuals with housing, benefits, healthcare, behavioral health treatment, employment and community resources. Coordinate services among healthcare providers, therapist, family members and community agencies. Maintain regular communication with individuals to monitor progress and identify emerging needs. Advocate on behalf of individuals across multiple systems. Maintain timely and accurate documentation. Participate in an on-call rotation, typically no more than once per month.

Minimum Qualifications

Associate’s degree in human services and 3+ years of mental health experience.
