Please note that this role is per diem with a rate of $50 - $60 per visit.
Provide direct patient care in accordance with the state Nurse Practice Act. Implement and follow individualized care plans established by the RN. Educate patients and their families on self-care techniques and diagnosis-related information. Document care accurately and in a timely manner. Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor. Use supplies and equipment safely, adhering to aseptic technique. Perform other duties as required for patient services. Graduate of an accredited practical or vocational nursing program. Active LPN or LVN license in California. 3+ years of nursing experience; home health or med/surg background preferred. Current CPR certification with AHA. Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes. Strong communication skills and a collaborative, patient-first mindset. Must be a graduate of an accredited nursing program with an active LPN or LVN license in California and current AHA CPR certification. Requires 3+ years of nursing experience and a valid driver's license with a reliable vehicle.
