Sentara Health is hiring a Full Time Care Coordinator for the Roanoke, VA area!

Hours: Full Time (40 hrs/wk)

Shift: Day (8am-5pm)

This position requires travel for in person face-to-face assessments in Roanoke,VA . Days without assessments scheduled employee can work remotely.

Experience

1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience (Required)

Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services (Preferred)

Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services (Preferred)

Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation (Preferred)
