Northeast Medical Group (NEMG), a member of Yale New Haven Health (YNHH), is seeking a Per Diem Palliative Care Physician to provide inpatient consultative services at Greenwich Hospital.

This position focuses on delivering high-quality, patient-centered palliative care to hospitalized patients with serious and complex illnesses.

The physician will work collaboratively with an interdisciplinary care team to manage symptoms, support goals-of-care discussions, and improve quality of life for patients and their families.

Key Responsibilities Provide inpatient palliative care consultations and follow-up care for hospitalized patients Perform comprehensive clinical assessments, including symptom burden, prognosis, and care needs Manage complex symptoms including pain, dyspnea, nausea, and psychosocial distress Facilitate goals-of-care discussions, advance care planning, and end-of-life decision-making with patients and families Collaborate with interdisciplinary teams (nursing, social work, chaplaincy, case management, and primary teams) to develop and implement individualized care plans Participate in family meetings to provide education, support, and alignment around care goals Coordinate care across the hospital setting to ensure continuity and appropriate transitions of care Document clinical encounters, treatment plans, and patient progress in the electronic medical record Provide education and guidance to hospital staff on palliative care principles, when appropriate Practice Environment Collaborative, hospital-based palliative care program within Greenwich Hospital Integrated with the resources and specialty network of Yale New Haven Health Physician-led, multidisciplinary model focused on quality, compassion, and patient-centered care Opportunity to work alongside experienced palliative care clinicians and subspecialists Why Choose Yale New Haven Health?

Qualifications
