About the role

The Case Manager is responsible for assessing consumer needs, developing service plans, and providing ongoing monitoring and advocacy to secure community resources. They must maintain detailed clinical records and collaborate with treatment teams to ensure effective delivery of case management services.

Responsibilities

Primary Job Functions: Clinical Responsibilities: Demonstrates the ability to assess the consumers’ needs. Identifies goals and plans for appropriate services. Provide appropriate linkage to community resources. Monitors consumers to secure placement in the least restrictive environment. Provides ongoing monitoring of consumers’ progress and needs. Advocates on behalf of the consumer to secure community resources. Effectively collaborates with the treatment team members and outside sources in the delivery of case management services. Evaluates the consumer for discharge planning. Provides family support and education as needed. Provides basis living skills instructions as needed. Effectively demonstrates the application of adult specific competencies. Supervision and Consultation: Seeks clinical supervision and consultation as needed. Regularly staffs concerns regarding consumers with supervisor. Accepts and employs suggestions for improvement. Actively works to enhance case management skills. Clinical Record Keeping: Documents in a timely manner per AltaPointe standards. Documents appropriate, complete, and comprehensive assessments. Documents measurable and specific goals. Documents in a clear and concise manner, addressing goals, interventions used, response to interventions, progress toward goals, and plan for follow through. Documents twice weekly staffing notes for consumers on caseload, per ACT standards. Documents case disposition, case closings, follow-up and referrals. Documents legibly. Documents using Avatar appropriately. Obtains signatures as required by supervisor for consumer contact. Maintains mileage reimbursement logs efficiently and with integrity. Case Load Management: Effectively manages caseload based on consumer needs. Responds in a timely manner to crisis situations. Makes face to face contact with consumers on caseload at least once weekly. Meets AltaPointe productivity standards Courteous and respectful towards consumers, visitors and co-workers: Treats consumers with care, dignity and compassion. Respects consumer’s privacy and confidentiality. Is pleasant and cooperative with others. Assists consumers and visitors as needed. Personal values do not inhibit ability to relate and care for others. Is sensitive to the consumer’s needs, expectations and individual differences. Is gentle and calm to consumers and families. Administrative and Other Related Duties as Assigned: Actively participates in Performance Improvement activities Actively participates in AltaPointe committees as required Completes assigned tasks in a timely manner Complies with AltaPointe policies and procedures Attends to required in-service training and other workshops Physical Requirements of the Job: Frequent standing, sitting, and walking which are in bursts of 5 to 10 minutes. There are times when this can be extended to pacing with the consumer (time could be undetermined) and at times sitting and standing can be extended with the employee having the ability to change positions as permitted by the observance and behavior of the consumer at that time. Occasional forward bending from the hips while kneeling to reach to the side. Occasional need to be able to assume a full squat/lunge from a standing position. Occasional need to maintain back extension while being on stomach with one’s own body weight on hips and legs. This position is maintained for several minutes. Occasionally, be able to put one’s own body weight on one knee and one elbow on the same side of one’s body. Occasionally, be able to kneel and reach to same side toward the floor. Lift up to 10 pounds occasionally during shift. Strong grasp is needed to use keys to open doors throughout the facility, and to maintain constant contact with a consumer during a take down procedure or with other hold techniques. Pushing/pulling force up to 25 pounds occasionally during a shift.

Qualifications

Bachelor’s degree in a mental health discipline; Post hire, must complete certification of case management training by Alabama State Department of Mental Health and Mental Retardation. Must have a valid state issued driver’s license and proof of current liability insurance. Knowledge in adult specialty, outlined below, to be attained within (6) six months of employment: Knowledge of psychiatric disorders in the adult population Knowledge of cultural diversity Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, and/or consumers with substance abuse issues as well as, to non-violent crisis intervention practice Knowledge of Psychiatric Rehabilitation principles Knowledge of available community resources Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders Knowledge of rules and regulations regarding appropriate benefit entitlements such as social security, Medicaid, and Medicare

Requirements

Candidates must hold a bachelor's degree in a mental health discipline and possess a valid driver's license with liability insurance. Post-hire, the individual must complete case management certification and acquire specific knowledge in adult psychiatric disorders and behavior management.
