What You’ll Do Conduct cognitive and emotional assessments to set/manage treatment plans Manage psychotropic medication and gradual dose reductions Collaborate with nursing home staff and SPC’s behavioral health team Participate in treatment team meetings (in-person and virtual) Deliver care both in-person and via telehealth when appropriate Maintain accurate, timely documentation in our EHR system

Licensed PMHNP or PA in the state of Louisiana Psychiatric/mental health experience required Experience with geriatric populations strongly preferred Spanish bilingual is a plus Must have a valid driver’s license and be comfortable traveling locally Adaptive, flexible, and committed to team-based care
