About the role

The Registered Nurse provides comprehensive healthcare and treatment to individuals with disabilities in community settings. Responsibilities include developing Individual Support Plans, monitoring medical conditions, and training staff on medication administration.

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.

We are seeking a Part Time Registered Nurse (RN) in Mercer, N.J. The Registered Nurse delivers care to individuals with disabilities in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. Hourly Pay Rate: $38.00

Responsibilities

  • Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
  • Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
  • Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
  • Train employees on the medical needs of assigned individual
  • Monitor health and medical conditions of individuals and refer for medical services as needed
  • Monitor the use of medications through medication administration logs
  • Ensure program documentation and billable records are completed accurately and timely
  • Participate with individuals in community, social, and recreational activities
  • Participate in circles of support for personal futures planning
  • Minimum Requirements
  • Valid R.N. license
  • Valid driver’s license with a satisfactory driving record
  • Ability to calculate figures related to medication dosages
  • Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community

Requirements

Candidates must hold a Bachelor's degree in nursing, a valid RN license, and a driver's license. A minimum of one year of experience providing nursing services to individuals with disabilities is required.
