As a member of our team, you’ll play a key role in providing quality home healthcare, where patients are able to remain independent at their place of residence in the healthiest and happiest state of being possible.

A Physical Therapist Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.

What you’ll be doing

Provide physician prescribed physical therapy. Improve or minimize residual physical disabilities of the patient. Return the individual to optimum and productive level within the patient’s capability. Participate with all other home care personnel in patient care planning. Documents all services provided, observations, client progress, and recommendations. Participated in interdisciplinary activities per Agency guidelines.

What we’re looking for

A passion to serve and help others live their best lives possible. Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association. Currently licensed in the state in which practicing. Minimum of six (6) months experience as a Physical Therapy Assistant. Two (2) years’ experience preferred in a home health setting Knowledgeable and experience with Medicare OASIS documentation Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record. Ideal candidate resides in Lorain County (Avon, Elyria, Lorain, etc.) Flexible work from home options available.

Requirements

Must be a graduate of an APTA-approved physical therapy assistant program and hold a current state license. Requires a minimum of six months of experience, with two years in home health preferred, and a valid driver's license with insured transportation.
