The PrEP Case Manager is responsible for providing high-quality, client-centered case management services to patients in the PrEP (Pre-Exposure Prophylaxis) program. The Case Manager will conduct screenings and intakes to determine program eligibility, in addition to developing individualized care plans. In addition, the PrEP Case Manager will provide HIV and STI risk reduction strategies, as well as medication adherence assistance. The PrEP Case Manager will have extensive client interaction and will collaborate with community partners in recruitment efforts to identify patients who are appropriate for PrEP services.
Bachelor’s degree in social work, Human Services, Psychology, or related fields required.
Minimum of 1 year of work experience in case management services.
Working knowledge in the field of HIV and STI’s, familiarity with community resources, the Harm Reduction model, ability to work in a fast paced, interdisciplinary team setting strongly preferred.
