About the role

Provide housing-focused case management and navigation to families experiencing homelessness or domestic violence to help them secure permanent housing. Coordinate resources, maintain HMIS documentation, and collaborate with community partners using a trauma-informed, participant-centered approach.

The Case Manager supports families experiencing homelessness, housing instability, domestic violence, or other immediate safety concerns through Just Compassion’s Family Shelter Program. This position provides housing-focused case management, housing navigation, stabilization support, and resource coordination to help families secure and maintain permanent housing.

The Case Manager works closely with participants, shelter staff, landlords, community partners, and service providers using a Housing First, low-barrier, harm reduction, trauma-informed, and participant-centered approach.

Essential Duties Provides housing-focused case management to families residing in the Family Shelter Program.

Supports participants in obtaining permanent housing through housing navigation, resource coordination, advocacy, and stabilization planning.

Maintains timely, accurate, and complete documentation in HMIS and other required systems.

Collaborates with shelter staff, supervisors, community partners, landlords, and service providers to support participant progress and continuity of care.

Ensures services are delivered in alignment with Just Compassion policies, Multnomah County requirements, HMIS standards, and applicable regulations.

Collaborate with the Family Shelter Waitlist Navigator to support equitable and timely shelter placement.

Support families during intake and orientation by explaining program expectations, timelines, available services, and participant responsibilities.

Serve as the primary point of contact for assigned families throughout their shelter stay.

Provide housing-focused case management, including support with housing search, document collection, applications, landlord communication, apartment viewings, move-in coordination, utility setup, furniture acquisition, and housing retention planning.

Support participants in accessing available rental assistance, including up to twelve months of assistance when eligible and available.

Develop individualized service plans that support participant goals, housing stability, and long-term self-sufficiency.

Provide ongoing advocacy, resource connection, stabilization support, and post-placement case management based on each family’s needs.

Participate in case conferencing, multidisciplinary team meetings, and service coordination meetings.

Maintain timely, accurate, and complete documentation in HMIS, including eligibility documents, case notes, service updates, and required participant data.

Monitor data quality and maintain accurate participant records.

Maintain confidentiality, professional boundaries, and participant dignity at all times.

Communicate proactively with supervisors regarding workload, participant needs, housing barriers, operational challenges, and emerging concerns.

Attend required meetings, trainings, and professional development opportunities.

Apply trauma-informed, culturally responsive, Housing First, low-barrier, harm reduction, and participant-centered practices in all interactions and services.

Perform additional duties as assigned in support of shelter operations and participant services.

Qualifications

Experience supporting families experiencing homelessness, housing instability, domestic violence, or other crisis situations. Strong communication, organization, documentation, and interpersonal skills. Ability to collaborate effectively with internal teams, community partners, landlords, and service providers. Ability to manage competing priorities in a fast-paced, crisis-responsive environment. Proficiency with Microsoft Office, Google Workspace, cloud-based systems, and client databases; HMIS experience preferred. Commitment to equity, cultural responsiveness, trauma-informed care, Housing First principles, and Just Compassion’s mission. Ability to maintain confidentiality, professional boundaries, and sound judgment when working with sensitive participant information. Valid driver’s license, proof of automobile insurance, and reliable personal vehicle for site visits and housing navigation. Ability to work occasional evenings and weekends to support program operations.

Preferred Qualifications

Experience with Coordinated Entry, shelter navigation, or housing-focused case management. Familiarity with the Multnomah County Homeless Family System of Care. Experience supporting families impacted by domestic violence, complex trauma, or other safety concerns. Advanced knowledge of HMIS or comparable data management systems. Bachelor’s degree in Social Work, Human Services, or a related field. Experience serving unhoused individuals, families, and other vulnerable populations. Bilingual or multilingual abilities, including Spanish, Russian, or other languages commonly spoken by participants. Previous experience providing housing-focused case management, supportive services, or Medicaid-funded services.

Requirements

Requires experience supporting families in crisis and proficiency with client databases and office software. A valid driver's license and reliable vehicle are required, while a bachelor's degree in Social Work or Human Services is preferred.
