About the role

The Spiritual Care Coordinator provides direct spiritual assessments and care to hospice patients and their families while coordinating with community chaplains. They serve as a key member of the interdisciplinary team to develop individualized plans of care and organize memorial services.

Empyrean Hospice hires Spiritual Care Coordinators for the provision of spiritual care services to patients and families/caregivers of Empyrean Hospice, either directly or through coordination of care with other spiritual counselors. Salary or hourly rate listed is the base for this role; increase commensurate with experience.

Position Overview – Spiritual Care Coordinator

  • Represent Empyrean Hospice with the utmost professionalism and compassion
  • Provide spiritual assessment of patients and families/caregivers in the hospice program and ensure appropriate services are rendered in a timely manner
  • Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
  • Serve as a member of the interdisciplinary group and provide consultation, education, and support to the interdisciplinary group on spiritual care
  • Provide direct spiritual care to patients and families/caregivers
  • Serve as liaison and support to community chaplains and spiritual counselors
  • Maintain records of spiritual care services utilization and related activities
  • Recruit community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
  • Provide for funeral or memorial services for patients as requested
  • Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team
  • Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers
  • Ensure standards of ethical business and clinical practice are prioritized

  • Position Qualifications
  • Professionalism, compassion, empathy, and a desire to help others
  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver
  • Self-directing with the ability to work with little direct supervision, yet also as member of a complete care team
  • Understand the philosophy of hospice care and of grief response and experience with counseling individuals and/or in groups regarding loss
  • Demonstrate comfort in an interfaith setting with ability to accept different lifestyles, cultures, beliefs, and values
  • Ability to network with community clergy and congregations
  • Possess strong communication skills, verbal and written, and organization skills
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Requirements

Candidates must hold a degree from an accredited seminary or school of theology (BD, MDiv, or equivalent) and have experience working with death and dying. A valid driver's license, insured vehicle, and the ability to work in an interfaith setting are required.
