As a Case Manager, you will play a vital role in supporting adults living with severe mental illness by connecting them to the services, resources, and relationships they need to thrive. This is a community-based position that combines advocacy, care coordination, and relationship-building to empower individuals on their recovery journey. You'll work alongside an interdisciplinary team of behavioral health professionals, medical providers, families, and community partners to ensure every person receives compassionate, person-centered care.
Requirements: What We're Looking For
Education Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or a related behavioral health field.
Equivalent education and relevant experience may be considered in place of a degree.
Experience Preferred qualifications include: Experience working in behavioral health, community mental health, outreach, or case management.
Experience supporting individuals living with severe and persistent mental illness or serious emotional disturbance.
Knowledge of recovery-oriented, trauma-informed, and community-based services.

The ability to work independently while collaborating effectively with a multidisciplinary team.
Excellent organizational and time management skills.
Experience coordinating care across multiple systems and community providers.

Strong documentation and computer skills, including Microsoft Office.

A commitment to ethical, person-centered, and strengths-based care.
Valid, non-expired driver's license. Reliable transportation with current comprehensive automobile insurance and a satisfactory driving record. Willingness to travel throughout the service area and transport clients as needed. Ability to successfully complete required background screenings, including criminal history and Kansas Child Abuse/Neglect Registry clearance.
