The LVN Case Manager organizes patient plans of care from admission to discharge and coordinates communication between providers, insurance, and pharmacy staff. They also provide clinical support by starting IVs and managing all aspects of patient care needs within an infusion center.
Oso Home Care Inc. is looking to hire a full-time LVN case manager.
Our main office is located in Irvine, CA where the 405 and 55 freeways meet.
Candidate must be IV certified.
This is an in office/in clinic LVN role supporting our home and clinic infusion patients and nurses.
Training to take place in our Irvine location and then transfer to our Long Beach, CA ambulatory infusion center.
Monday through Friday, 8: 00 am – 7:00 pm (8 hours shifts: varying starting/closing shifts to support business needs)
1. Experience with PICC lines, implanted ports, and IV therapy within LVN scope of practice. 2. Experience starting IV’s and drawing blood. 3. Spanish speaking is a plus but not required. 4. Prior experience working in a fast-paced clinical support environment.
