This role is ideal for someone passionate about recovery, community outreach, and helping individuals overcome barriers to care.

Re-engage individuals who have fallen out of care and support their return to treatment and recovery services. Maintain an active outreach and engagement caseload. Serve as a liaison between clients and clinical staff. Facilitate non-clinical life skills and wellness groups. Conduct community presentations and prevention education. Build partnerships with referral sources and community organizations. Provide discharge follow-up and aftercare support. Travel statewide to support screenings and outreach events
REQUIRED: High school diploma or GED REQUIRED: Valid driver's license with 2 points or less REQUIRED: Must Have Own Vehicle Available for Use - Statewide Travel
